(2) A Proof of Occupancy (Lease Agreement, Property Tax Bill, Recent Closing Papers, or a Residency Attestation Form)
(3) TWO documents from the following: Utility Bill, Vehicle Registration, Voter Registration, or Recent Credit Card Statement

This system will automatically verify the presence of your name and address. Please ensure all documents contain the same name as you have entered below.

You only need to complete this form once per household.
